透過引人入勝的敍事手法，《黑手黨 2》帶領玩家進入黑暗且無情的黑幫世界。無論是服裝丶車輛丶音樂還是廣告，制作團隊試圖完美重現 40丶50 年代的美國城市風情。在這充滿生氣的複古城市中，有著個性鮮明且逼真的角色，並藉由爆炸性的槍戰以及緊張刺激的飙車場面，以及迷人的故事內容，讓玩家輕松回到那個年代，融入宛如電影丶好萊塢式的遊戲體驗。
義大利裔美國人 Vito，是貧窮移民的後裔，一心想要擺脫孩童時期的貧困生活。在街頭上，Vito 得知加入黑幫是他這種人追求財富及獲得尊敬的唯一途徑在。脫離他父親那種艱苦生活的渴望驅使下，他夢想成爲一名「黑手黨成員」。一輩子沒犯過什麽罪的 Vito，與他孩提時期的好友 Joe，攜手深入組織犯罪的世界。他們將同心協力向黑幫證明自己的實力，並試著在街頭揚名立萬。從行竊和偷車這種低階差事開始幹起，Vito 和 Joe 快速地在黑幫家族中竄升……，不過，這些自命不凡家夥的生活，卻不像外表那般地光鮮亮麗。
